READ THIS FIRST: This form is for organizations and individuals who have already signed up with Phoneraiser. If you have not already, please visit our registration form.

If you are already registered with phoneraiser, simply complete the form below. You will see a link (or links if you entered more than one box) to a fedex label in pdf form immediately after you submit the form. Follow this link, open the file, and print out the label. Tape this label on your box and drop it off at any FedEx or Kinko's location or schedule a pickup by calling 1-800-463-3339.
